According to SAMHSA, an intensive outpatient program, or IOP, sits between standard weekly outpatient therapy and more intensive levels of care within the continuum of care, such as partial hospitalization or residential treatment, providing several hours of structured clinical time each week while you continue living at home.
At Clarity Healthcare, every plan starts with an initial clinical assessment. That same assessment separates IOP from a Partial Hospitalization Program. PHP involves more clinical hours and a higher degree of structure, as a step down from inpatient care. IOP fits people who are stable enough to manage most of their week on their own. Under the ASAM Criteria, a structured clinical evaluation determines which level of care fits your situation. We weigh components such as symptom severity, safety, and the stability of your home environment, and build your individualized treatment plan from there.

Our substance use disorder IOP is built for people working through alcohol or drug use, including fentanyl and methamphetamine, where a mental health condition isn’t the primary concern. Treatment focuses on understanding triggers, rebuilding routines that support recovery, and relapse prevention, along with medication-assisted treatment when it’s clinically indicated.
Dual diagnosis IOP exists for people managing a mental health condition and a substance use disorder together, since each tends to affect the other. Rather than two separate plans, it’s one integrated plan built by a clinical team that treats both as connected, with therapy, medical support, and case coordination working from a common understanding of what you’re managing.
What a Week in Our Intensive Outpatient Program Looks Like
Our IOP in Washington State runs on several coordinated sessions each week, set at defined days and times, rather than a single weekly appointment. That regularity, more than any particular number, is what separates this level of care from standard outpatient therapy.
The number of sessions in your week comes from your individualized treatment plan. Someone in an earlier, more acute phase may need a heavier schedule than someone stepping down toward lighter support. Sessions mix group and individual therapy.
Psychiatric support is built into the same weekly rhythm when it’s part of your plan. We schedule sessions in the morning and evening, because working around a job, classes, or family is part of what this level of care is meant to do. You can attend online or in person.
What Treatment Includes in Our Intensive Outpatient Program
Individual therapy is your dedicated time with a therapist to work through what’s specific to you. In contrast, group therapy brings you alongside others working through something at a similar level of intensity. Neither replaces the other: one gives you focused attention, the other gives you a sense that you’re not managing this alone.
Within both formats, your clinical team draws on evidence-based approaches suited to your assessment. This can include cognitive behavioral therapy and dialectical behavior therapy, selected based on what you’re working through, not a fixed curriculum. When medication is part of your treatment, we build psychiatric support and medication management directly into your plan. That could mean an initial evaluation, ongoing management, or cooperation with a prescriber who already knows your history.
Preparing for Life Beyond IOP in Washington State
At Clarity, your clinical team starts working on relapse prevention and long-term planning early in your sessions. The plan you leave with has already been tested, not handed to you at the last minute. That work looks different depending on your track. Long-term recovery planning carries that same specificity into life after IOP. Your plan may include continuing support such as outpatient therapy, a support group, or ongoing medication management. If your clinical picture calls for a different level of care, lighter or more intensive than IOP, we handle that referral through direct care coordination with the receiving provider. We share your history with your consent, so you do not have to start over.

Choosing Between Onsite and Remote IOP

For our virtual IOP, we look at whether you have a stable home environment, a private space to speak openly during sessions, and reliable internet that can support real clinical work. If you are managing a household in crisis or do not have dependable internet, our in-person IOP may be a better fit until those circumstances change. If your situation changes later, we revisit format the same way we revisit your track and your schedule.
